How will this be achieved?

London ASC Centre aims to provide a rehabilitative programme of interventions over 2 years covering the following areas:

  • Social inclusion

community based activity programmes that develop integration skills for this individual e.g. visiting places of local interest, orientation to the local area and what it can offer for the individual, social activities that are meaningful for the individual. This can then be measured using a Keeping Track tool that staff will complete daily to demonstrate how frequently and meaningfully individuals are being included in their locality.
  • Behavioural modification

individual intervention programmes delivered by trained staff that are based on function of behaviour, developing new skills and schedules of reinforcement that will produce effective behaviour change. ABC analysis will demonstrate function of behaviour and behaviour support plans will enable reductions in behaviour to be recorded and be monitored monthly. Individuals will also have specific physical intervention plans written in accordance with their care plans and risk assessments.
  • Development of daily living skills

individual independent living skill programmes based on task analysis with the addition of Keeping Tracks tools in order to monitor outcomes around cooking, cleaning, laundry skills, budgeting, shopping etc. The forms will establish a baseline and then an increase over time. This will also include Occupational Therapies and sensory activities such as yoga, hand and foot massage etc
  • Engagement in further education and/or supported employment

Individuals will have access to on site educational programmes run by Qualified Special Educational individuals, if this is meaningful to them as an individual. The site will offer a variety of different educational programmes including, IT skills, numeracy and literacy, finance management etc and outcomes will be demonstrated by graphical representations of increases in educational skills.
